[CALIBK12] Student Survey

Instead of using our district's standard performance evaluation, I am
going to do an Action Research.  I'd like to survey 3-4 teacher's
classes with students who are below grade level in reading regarding
their interest/use of libraries, what they'd like to find in libraries,
their attitude toward pleasure reading, etc.  Later in the year I would
survey them again after doing book talks and activities with them.  Does
anyone have suggestions for questions or know of a survey that already
exists? Thanks.
