[CALIBK12] Library Collaboration form

Good Morning,

Does anyone have a classroom teacher and librarian collaboration form  
for elementary that they would be willing to share that really has  
worked well for your school?  We've just consolidated two campuses,  
K-3 and 4-6,  over the summer and I can't put my hands on them yet.   
We're waiting for new, larger library space in January. However the  
library became a storage room during the move and I am still putting  
the room back together.  My 4-6 library is still boxed up and with me  
in the library.  I don't trust it in an outdoor storage container.
