[CALIBK12] Create Curriculum Survey

Hi All
Well, it looks as though there are more people who would LIKE to do a curruculum survey, than those who actually do, or at least are willing to share.  So it seems to me that this would be an excellent opportunity to use those 2.0 skills we have acquired, and work together on creating a curriculum survey.  So I have added a page on my Readervation wiki and hope you will come up with at least one question you would like to ask your staff.  
 
Of course, as with all wikis you have the ability to edit and the outcome will be something we can all use.
