We also use these citation forms at my middle school. I teach the 6th graders how to fill them out and where to find the necessary information, and the 7th and 8th grade teachers expect our students to know how to use them, often requiring that they turn in the forms along with their research projects. I always give Deborah credit for creating them--they are a great tool!

Scroll to the "Sources" section for the instructional PowerPoints on sources and citation forms.
Just a quick note that I use these forms for citation instruction so that students can directly engage a source and actually learn the order and punctuation of each MLA item in a citation. I only use online citation makers when the students have experience citing sources.

I am showing fourth graders how to create a bibliography using easybib. Since they don't have email accounts I simply had the first group copy and paste from easybib into a Word document and print. Even though I wrote out a step by step tutorial it was a lot of "steps" for them to follow. Does anyone have an easier idea or way of showing them how to site various soures?
