[CALIBK12] Site Library Plan

Hello all,
 
I need to produce a site library plan. While our district has one, we do not. Is there anyone out there that has one that I could peruse? It would be somewhat better if:
 
a. It was for a middle school; and 
b. It was for a Title 1 school;
 
However, I am happy to use whatever I can get! :)
