[CALIBK12] Student Book Reviews

Attached are the forms we use for our book review blog. There is the application to join the blog, a format sheet so the reviews placed on the blog are consistent in format, and a review template. Hope this helps.

Hello. I was wondering if anyone had a short book review form/template  
that you have students fill out to recommend particular books. I want  
to begin adding them to our web site. Are there certain guidelines you
