[CALIBK12] school surveys

Good morning - Bear with me as I am brainstorming here.  I want to
survey our school community regarding the library, library services and
staff.  I am thinking of using an online survey generator and putting
the link on the school webpage and advertising with teachers.  I would
like to include questions about what they would like to see in the
library; whether it be books, magazines, etc.

 

Does someone have a template or a sample that I could look at?  Do you
have any advice related to surveys/polls?  Have you had success or found
them beneficial at all?
