[CALIBK12] Reading Interest Survey

I use this one.  It seems to work really well, especially for the reluctant reader.  It helps me find books on their interests that they never thought the library would have or books would be about.

I need a Reading Interest Survey for a lesson next week. I need in on paper, not the Internet, though the Internet would work if I can print it out. I did search the archives, but couldn't find anything. TIA
