[CALIBK12] Query: High School Library Survey

As part of our WASC process we are forming a Library Advisory Committee to lay out a vision for our library for the next 6 years.  We would like to conduct an online survey of our students, teachers, staff, and parents to help us determine the services and resources that would be most wanted and needed by our community.  I would love to have some examples to show the committee to help us with this first step.
