I just sent this reply to Linda, but since Thomas asked, I'll post to the
group. May be helpful to others and an idea to discuss (soon!) with your
principal if there's no policy in place, or current policy doesn't work.
This has been our policy/procedure in the past, but with a new principal
this year, I will need to discuss with her. I would also advise checking
with the district to see if there is a district-wide policy (principal would
probably know or could find out). -JL

"No particular form - I remind them about it during the year (just before
the trimester change is a good time) and then at the end of the year (if it
hasn't turned up) it is reported to the office/principal, and our district
docks their paycheck if they don't pay in advance by check. My principal has
two check-offs on the end-of-the year checklist that every classroom teacher
has to turn in. One is a check-off that all their (homeroom) students have
turned in or paid for their library books, the other is that teacher has
cleared his/her library account with me. I initial and date that one (they
send a runner with the paper to me, or put in my office box) but after I do,
they can't check-out any more books. Just knowing it is on the list usually
gets good results. I would work with the principal and set a date by which
they get their books in, or paid for. After that date, they know you are
required to submit a list to the principal. This way the principal sets the
tone and policy with their authority."

As an aside, do people charge staff members for lost library books? I never
have, but it is also a very rare occurrance.
