[CALIBK12] Permission Slip For Books

Does anyone have a good permission slip for books they can share and email to me? I'm here until late July, and want to re-work my YA section, and have permission slips at the ready for students trying to check out YA books. I will hand them out with a page of book reviews for the parent's review, and they can return the slip signed if they approve of their child reading the book.

I have a K-8 library, and I have 3rd graders trying to check out Twilight, or 5th graders trying to convince their parents that Hunger Games is harmless. I'm not one to censor books, or prevent a child from exploring new titles or broadening their horizons, but I am a tad weary of parents yelling at me or preaching how their innocent students have never read such books. If anyone has experiences to share and wisdom to bestow upon a librarian, I would love to hear from you.
